CONTEXT AND RATIONALE

Vaccination has had a number of successes in Chad, particularly in the eradication of poliomyelitis with the obtaining of the status of polio-free country confirmed by the Regional Certification Committee in June 2016, the absence of outbreaks of meningitis A thanks to the organization of a quality mass campaign with MenAfrivac in 2012.

However, the performance of the routine EPI remains weak with a vaccination coverage rate of 22 percent of children aged 12 to 23 months fully vaccinated according to the 2017 Vaccine Coverage Survey with a disparity between regions. The major causes of the poor performance of Chad’s vaccination program are, among others: poor effective implementation of vaccination strategies, insufficient competent human resources, insufficient supply of quality vaccination services, inadequate approaches to demand generation, inadequate vaccine logistics and management, low availability of quality data and their use for action, weak program management and coordination , the lack of accountability of actors vis-à-vis the results of vaccination and health as well as the absence of a multisectoral approach.

The national assessment of the cold chain and logistics conducted in 2010 revealed very significant shortcomings and gaps and resulted in a cold chain and logistics revitalization plan developed and implemented since 2011. Effective Vaccine Management 2015 showed persistent shortcomings, in particular the late start-up of the cold rooms of the 4 sub-national depots. The central level depot is currently under the responsibility of a logistics manager assisted by 4 logisticians and 2 storekeepers with sometimes the use of day laborers to accomplish certain tasks. In 2018, to ensure the operation of sub-national depots, the Ministry of Public Health provided each depot with a vehicle and staff consisting of a manager and his deputy, a manager and a driver. .

The immunization program benefits from the following opportunities:

• A strong commitment of the Chadian government at the highest level of the State through the Head of State with the provision of domestic resources for the financing of vaccination activities, giving priority to the purchase of vaccines and injection materials

• Technical and financial support from many partners including Gavi, WHO, UNICEF, BMGF, World Bank, Rotary International, bilateral cooperation and many other partners who fund the EPI through various mechanisms.

• The upcoming funding of the cold chain platform optimization plan (CCEOP) by GAVI.

However, Chad’s EPI faces many challenges, including:

• Lack of accountability and low commitment of agents at all levels

• The financial crisis having a negative impact on the level of the Government’s financial contribution to health and EPI activities

• The persistence of epidemics, particularly of measles, due to low vaccination coverage

• Low coverage of districts with cold chain meeting WHO standards (46%)

• The less than optimal functioning of sub-national warehouses and cold chain devices in districts and health centres.

In accordance with its mandate, UNICEF supports Chad in the acquisition of cold chain equipment, their installation and their maintenance. The Program Support Justification document (JSP) including Chad’s CCEOP was accepted by GAVI’s Committee of Independent Experts in December 2018. Its implementation will require strong support from the EPI. In addition, a process of solarization of subnational repositories is underway, with material for Abéché being acquired.

It is within this framework that UNICEF is recruiting a National Technical Assistant to help the country in the implementation of the CCEOP and the solarization of subnational deposits.

GOALS

Provide technical support to the EPI team for the implementation of the CCEOP. The National Technical Assistant will be more specifically responsible for:

• Follow up on orders for solar equipment as part of the CEOP

• Coordinate and supervise the installation of cold chain equipment and their maintenance

• Provide technical support for the solarization of the sub-national deposits of Abeche, Moundou and Sarh

• Support the implementation of the improvement plan resulting from the EVM

• Support the EPI logistics team in the monitoring and maintenance of cold chain equipment (preventive and corrective maintenance) at central and decentralized level

• Support remote monitoring of cold chain equipment temperatures

• Train health personnel in the management and maintenance of this equipment.

TASKS AND RESPONSIBILITIES

The National Technical Assistant for the CCEOP and the solarization of subnational depots will contribute to the implementation of the CCEOP and to improving the quality and management of the cold chain.

The main results expected from the National Technical Assistant for the CCEOP and the solarization of subnational deposits are as follows:

• At least 90% of the cooling equipment received is installed within 3 months of receipt and is functional

• Capacities of MSP logisticians and technicians strengthened in the field of cold chain and vaccine logistics.

• Cold chain equipment inventory report updated and available

• Inventory report of fixed assets at all levels and office automation available in November 2019

• Abéché subnational repository solarization report available

• Study report on the feasibility of the solarization of the three remaining subnational deposits is carried out

To qualify as an advocate for every child you will have…

Training:

• Have a Master’s degree issued by a University recognized by UNESCO in the field of the cold chain, medical engineering or health logistics.

Experiences

• Have a professional experience of at least five years in the installation of solar equipment

• Having experience in an emergency situation is an advantage.

• Experience in healthcare supply chain management is an advantage

• Be able to carry out regular missions to monitor activities inside the country.

Language

• Mastery of French and knowledge of local languages: Chadian Arabic, Sara, etc.

Required Skills

• Mastery of word processing and spreadsheet software (Word, Excel) and PowerPoint

• Ability to work in a team, empathy and flexibility, organizational skills

NB: Having experience of working with non-governmental organizations, United Nations agencies (UNICEF in particular) is an asset.
